  1.  Late Registration accepted if space permits.
  2.  Included Meals: Sept. 11, Lunch & Dinner, Sept. 12, Breakfast
  4.  When form is complete, click submit to send. You will automatically receive an invoice.
     Print it from your browser and submit to school/district for processing.
  5.  Attending Advisors DO pay registration fee.

 ADVISOR COMMITMENT
 
 By submitting this form you agree to the following:
1. Each student has completed the SkillsUSA Medical Liability and Release form and you have completed the
online Statement of Assurance.
2. I will bring a completed copy of each Medical Liability and Release form to the conference and turn-in at registration.
3. Substitutions will be accepted prior to the conference. Additions to your registration will be accepted if space permits--contact the SkillsUSA--Arizona office with questions.
4. All invoices are subject to the Payment Policy.
